Requirements for Applying:

Most Current Tax forms:

● ​Federal 1040 Tax Form ● W2 Forms- if available ● ​IRS 4506-T Form- (Signed & turned in if no tax return for current year)

State Eligibility Letter: 

● ​Proof of Eligibility for School Lunch ● State Benefits Eligibility Award

Additional Requirements:

● ​Current Paycheck Stub ● Letter from Employer
